Registered Nurse, Case Manager

  • Full time
  • Phoenix metro
  • Reports to the Director of Clinical Services

You carry your own caseload. You admit new patients, build the plan of care with the family and the physician, and keep every member of the team working from the same plan. When symptoms change, you are the one who decides what happens next.

What the work is

  • Admit patients to hospice and assess what they and the family need
  • Build and update the plan of care at team meetings, based on what you find
  • Manage pain and other symptoms using both medication and comfort measures
  • Coordinate the nurses, aides, chaplain, social worker, volunteers, equipment and medications for your caseload
  • Supervise and support the LPNs and aides caring for your patients
  • Teach families what to expect as the illness progresses

What we ask for

  • Current Arizona RN license in good standing
  • One to two years of nursing in an acute care setting
  • A year of home health, hospice or comparable experience, preferred
  • Comfortable working alongside social workers, chaplains, aides and volunteers
  • On call by schedule, with calls returned inside 20 minutes
More about this role

You manage each patient’s medication profile so the list stays short and aimed at symptom relief rather than growing on its own. You make sure care is delivered at the right level, and that the primary care physician and the medical director stay in the loop. You triage scheduling and symptom calls from the team, take part in the performance reviews of the LPNs and aides on your caseload, help orient new clinical staff, and contribute to quality audits. Covenant provides orientation and training, in house or through outside seminars, and records it in your personnel file.

Apply for this role

Nine questions, about two minutes. Have your Arizona RN license status and a resume ready. If your experience fits, we will call you and send a short credential form before we meet. Fields marked * are required.

Licensed Practical Nurse, weekend on call

  • Full time, on call
  • Friday 5 PM to Monday 8 AM
  • Phoenix metro

Weekends are when families feel most alone with this. You are the nurse who answers. You take the calls, make the visits, and manage symptoms through the night, with an RN, nurse practitioner or physician behind you the whole time.

What the work is

  • Answer on-call calls and respond within 15 minutes
  • Make on-call visits, evaluations and admissions as assigned
  • Manage pain, bowel and skin or wound problems under the plan of care
  • Use the medication formulary first for symptom control
  • Record what you find and report changes to the on-call team and the patient’s case manager
  • Teach families what to watch for and what to do next

What we ask for

  • Current LPN license in good standing with the State Board of Nursing
  • At least two years of experience as an LPN
  • A year in hospice or long-term care, preferred
  • Reliable transportation and comfort driving to homes across the metro
  • Able to lift, push or pull up to 50 pounds on occasion
More about this role

You work within the LPN scope defined by the Board of Nursing, under the supervision of the Director of Clinical Services or a designated RN, nurse practitioner or physician. A clinical backup is assigned to you at all times, and you notify that person when a patient’s condition changes enough to change the plan, and when a patient dies. You also help with quality work, including chart audits reported back in good time, and you attend staff and nursing meetings. Covenant provides orientation and training, in house or through outside seminars, and records it in your personnel file.

  • Take and record blood pressure, temperature, pulse, respirations, weight and mid-arm circumference
  • Safe transfers, repositioning and range of motion
