What this policy covers

This policy explains what happens to information you give us through covenanthospicecares.com. It does not cover the medical record we keep as your hospice provider, which is governed by our notice of privacy practices.

What we collect

• What you type into a form. Our care inquiry, referral, volunteer, and contact forms ask for a name, phone number, email address, and a short description of your situation. Referral forms from clinicians also ask for patient identifiers.
• Basic technical information. Like most websites, our server records the page requested, the time, the browser type, and an IP address.

Why we collect it

We use what you submit to answer your question, to reach you about care, and to route a referral to the right clinician. We do not sell it. We do not rent it. We do not trade it for advertising.

No ad tracking on care forms

Pages that carry a care inquiry, referral, or contact form do not run advertising pixels, remarketing tags, or third-party trackers. Health interest is not something we are willing to leak to an ad network, and someone reading these pages at two in the morning should not be followed around the internet afterwards.

Who sees your information

Staff who need it to respond to you, and the vendors who host this website and deliver our email and forms. Those vendors are bound by contract to protect it and to use it only for the service they provide us. We disclose information otherwise only when the law requires it.

How long we keep it

Inquiries that do not lead to care are kept only as long as needed to respond, and then deleted. Information that becomes part of a patient record is retained under the schedule described in our notice of privacy practices.

Cookies

This site uses only the cookies needed to make pages load and forms work, and no analytics product.

Children

This site is written for adults. We do not knowingly collect information from anyone under 13 through this website. Families arranging care for a child give us that information themselves, as the child’s parent or guardian.

Links to other sites

Where we link to another organization, such as Medicare or the VA, their privacy policy governs what happens there, not ours.
