Small teams, managed caseloads, and time to actually care for someone.
Three roles are open now, all of them in the homes and facilities we serve across the Phoenix metro. Choose one to read the work and apply. If none of them fit and you still want to do this work, choose the closest one and tell us what you bring.

You carry your own caseload. You admit new patients, build the plan of care with the family and the physician, and keep every member of the team working from the same plan. When symptoms change, you are the one who decides what happens next.
What the work is
What we ask for
You manage each patient’s medication profile so the list stays short and aimed at symptom relief rather than growing on its own. You make sure care is delivered at the right level, and that the primary care physician and the medical director stay in the loop. You triage scheduling and symptom calls from the team, take part in the performance reviews of the LPNs and aides on your caseload, help orient new clinical staff, and contribute to quality audits. Covenant provides orientation and training, in house or through outside seminars, and records it in your personnel file.
Nine questions, about two minutes. Have your Arizona RN license status and a resume ready. If your experience fits, we will call you and send a short credential form before we meet. Fields marked * are required.
Weekends are when families feel most alone with this. You are the nurse who answers. You take the calls, make the visits, and manage symptoms through the night, with an RN, nurse practitioner or physician behind you the whole time.
What the work is
What we ask for
You work within the LPN scope defined by the Board of Nursing, under the supervision of the Director of Clinical Services or a designated RN, nurse practitioner or physician. A clinical backup is assigned to you at all times, and you notify that person when a patient’s condition changes enough to change the plan, and when a patient dies. You also help with quality work, including chart audits reported back in good time, and you attend staff and nursing meetings. Covenant provides orientation and training, in house or through outside seminars, and records it in your personnel file.

You are in the room more than anyone else on the team. A bath, a clean bed, a careful turn, an unhurried hour. Families remember the aide.
What the work is
What we ask for
You attend team and staff meetings, follow infection control and safety procedures, and help teach families how to care for someone at home and how to look after themselves while they do it. You take part in grief support activities as you are able, and in community education when asked. You keep patient information private, uphold the Hospice Code of Ethics, and complete Covenant’s orientation and training, which is recorded in your personnel file.

Call anyway. We would rather know a good clinician exists before we need one.
